1. Eligibility Criteria

Hong Kong permanent residents with Chinese citizenship are eligible for applying and obtaining the “Mainland Medical Practitioner’s Qualification Certificate” through accreditation. The applicant should, in accordance with the “Law of the People’s Republic of China on Medical Practitioners” and related regulations, fulfill the following criteria:
1) Hong Kong permanent resident who has obtained relevant medical practicing qualifications and registration in Hong Kong for at least 5 years. (by 31st December 2007)
2) Acquired Specialist practitioner qualification certificate
3) Currently employed by a recognized medical institutions in Hong Kong SAR

2. Practitioners of the Following Medical Professions in HKSAR are Eligible for Applying for Mainland Medical Practitioner’s Qualification Certificate

According to pplement V> signed on July 29, 2008, those Hong Kong doctors could obtain the certificates through accreditation are: Western medicine and Dentistry.

3. Documents to be Submitted and Related Requirements

a. Documents to be Submitted via Post:

1) Application Form for Mainland Medical Practitioner’s Qualification Certificate (downloadable from the website after submission online, and duly signed by the applicant )
2) Two two-inch half-length color photos taken within 6 months showing a full front view of the face with no hat
3) A photocopy of the applicant’s Hong Kong Permanent Identity Card ( both sides)
4) A photocopy of the applicant’s academic certificate or diploma in the relevant category of medical qualification applied for
5) A photocopy of a valid practicing certificate issued by the Hong Kong SAR
6) A photocopy of a certificate issued by the Hong Kong SAR stating that the applicant has been included in the Specialist Register of the relevant specialty in the relevant category of medical qualification applied for
7) A copy of proof of current employment or practice registration in recognized medical institutions in Hong Kong SAR
8) A copy of proof of no misconduct in the professional respect during practice
9) A copy of proof of no criminal records
10) A copy of bank payment receipt for payment by bank transfer or a crossed cheque payable to “Guo Kang Pharmaceutical & Medical Supplies Limited “(For details, please refer to 7. Application Fees)

b. Requirements on the Documents:

1) Documents listed under (3) to (9) above should be notarized by notary agency in Hong Kong SAR or China-Appointed Attesting Law firms.
2) Where a document is not in Chinese ( either Simplified or Traditional), it must be accompanied by a Chinese translation certified as a true translation
3) Application should be submitted by registered mail or EMS
4) The application form, downloadable from the website, needs to be duly completed and signed by the applicant
5) Supporting documents should be printed or photocopied on A4 papers, and presented in sequential order as listed under the “Documents to be Submitted via Post” section
6) Application documents will be mailed back to the unsuccessful applicant’s address while those of successful applicants will not be returned. The applicant is advised to retain original or photocopy of the documents.

4. Enquiries

Guo Kang Pharmaceutical & Medical Supplies Limited (a company set up by the Ministry of Health in Hong Kong) is the sole agency appointed by the Ministry of Health of the People’s Republic of China to accept applications from Hong Kong practitioners who wish to obtain the Medical Practitioner’s Qualification Certificate. Applicants can login and apply on line on the website of “Application Service for Hong Kong Doctors to Obtain Mainland Medical Practitioner’s Qualification Certificate”. The application form, once submitted online, is downloadable and should be sent together with supporting documents to the following address:

5. Preliminary assessment

During preliminary assessment, the applicant’s qualification, as well as the supporting documents and application form will be reviewed. The applicant will be notified if additional supplementary information is required. Applications which pass the preliminary assessment will be delivered to the Mainland health administrative departments for final approval.

6. Assessment by Mainland health administrative departments & Delivery of certificates

A. The Mainland health administrative departments are responsible for the final assessment and approval of the applications. Successful applicants will be granted the <Medical Practitioner’s Qualification Certificate> issued by the Ministry of Health. The certificate will be post to the applicant’s registered address.

B. The health administrative departments will issue “notification of application result” to those applicants who fail the assessment to obtain the certificate, and the application package will be returned together with the notification by mail.

7. Application Fees

The application fee for the certificate is HKD 10,000 per applicant. Upon submission of the application package by mail, the applicant should pay in full the application fee of HKD10, 000 in either one of the following ways,

b. By cheque:

The cheque should be enclosed in the application package, crossed and made payable to the " Guo Kang Pharmaceutical & Medical Supplies Limited ".

An official receipt will be mailed together with the application results to the applicant.

8. Submission of Application and Processing Time

Completed and signed application form and all supporting documents should be submitted by the applicant via post. Upon receipt of all the documents and after preliminary assessment, the application package will be assessed by the mainland health administrative departments. The applicant will be notified of the application result by mail. It will normally take 100 working days to process the application upon receipt of all the required documents. The application would not be processed unless all the required documents and application fees are received.

9. Those obtain <Medical Practitioner’s Qualification Certificate> who want to apply for a <Medical Practitioner Practicing Certificate>, please refer to the <Law of the People’s Republic of China on Medical Practitioners> or inquiry to the local health administration department.
